If you are feeling unwell, isolate immediately and call 811 or your local health authority for direction.
Thruster Energy Corp. Employee, this questionnaire must be submitted for evaluation.

NOTE: If you answer “YES” to any of the below questions, additional precautions may be required prior to your return to work. Do not return to your worksite prior to receiving written clearance from Thruster Energy Corp Health & Safety team.

•Alberta Health Services has developed an online self-assessment tool to help determine what measures are required for your situation.
*Close contact is defined as a person who:
•Has provided care for an individual (e.g.: healthcare worker, family member, caregivers)
•Has been in close physical contact with an individuals without consistent and appropriate use of personal protective equipment
•Has lived with or had close prolonged contact (within 2 meters) with the individual while they were infectious.
